Our year-long housing program provides a stable foundation for men reentering the community after incarceration, regardless of their conviction history. Designed to support a gradual and sustainable transition, the program offers individuals the time and resources needed to heal, grow, and rebuild their lives. By fostering personal development and independence, we empower our clients to successfully reintegrate into society with confidence and stability.

Program Highlights

Men releasing from prison are able to access immediate housing the day of release, with supportive services that include:

  • A fully furnished one-bedroom apartment with all necessities for independent living.
  • An assigned case manager offering on-site support and life skills coaching.
  • Bus passes, groceries, clothing, hygiene items and other items based on individualized need.
  • On-site mental health assessment and access to individual therapy sessions.
  • On-site nursing services provided by an RN to support medication management, daily health monitoring and support meeting physical health needs.
  • Assistance with job placement, including soft skills coaching, resume building and referrals to several employment partners.

What to Expect

The Foundations First for Men program offers access to various support services. These program offerings build upon the foundation of a safe, stable place to live, and are focused on achieving physical, mental and emotional wellness for a healthy, sustainable future.

    Clients of the Foundations First Program can choose from weekday, evening, and weekend group sessions that focus on shared experiences, healthy coping strategies, life skills, and recovery. Led by peers with lived experiences, these groups create a safe and supportive space for connection and growth.

    Every client enrolled in the Foundations First for Men program is assigned to a case manager who helps develop an individualized care plan- a personalized roadmap outlining goals and strategies for success. Clients may also request individual therapy sessions with our Licensed Professional Counselor, and on-site nursing services provided by an RN support their journey toward improved physical health.

    Eligibility

    Please review the following criteria for eligibility in our Foundations First Housing for Men program.

    • Eligible candidates for CJM’s housing program must be incarcerated at the time of referral, which must be submitted by Institutional Parole Officers (IPOs), caseworkers, or other prison staff.
    • Applicants must identify as male and be released under supervision with Probation & Parole—completed sentences or 12/12 releases are not eligible.
    • Candidates must have medical and mental health scores of M1/MH1 or M2/MH2, with no parole violations on their current sentence and no significant history of multiple parole or probation violations.
    • Sex offenders must have completed the Missouri Sex Offender Program (MOSOP), though availability for these units is limited.
    • Additionally, candidates must not have any other approved or pending home plans outside of Missouri, outstanding warrants, or pending cases.
    • A history of good institutional adjustment is required, including active participation in programming such as ICVC or Anger Management and maintaining employment while incarcerated, with few to no conduct violations in the past three years.
    • Candidates must not receive SSI/SSDI or plan to apply for it.
    • Finally, applicants must have a Board-approved release date within six months, with preference given to those closer to release.
